NEW PATIENT INFORMATION
Welcome! We're looking forward to a lasting partnership with you. As you might expect, there's paperwork to do. You'll save time on your first visit if you take a few minutes now to gather the items you'll need for your first appointment. Below are a few of the necessary forms/paperwork that will be a part of your visit, some of which you should bring with you to the office. Others, we will provide for you. In either case, please plan to arrive 20 minutes before your scheduled appointment to complete your registration. If you are having the in-office Permanent Sterilization ESSURE procedure, please check with our staff for your arrival time. Thank you!

Just one more thing... If you were referred to Dr. Walden or Dr. Stimage by another physician, or you are a new patient with us, please have your physician send a copy of your medical records to our office:

Included in this will be the ability to prepare for your visit via a secure online portal. Please be patient with us during this transition as we prepare to better serve you!
MAKING AN APPOINTMENT
For your convenience, we book appointments during regular business hours Monday through Thursday 9:00am - 5:00pm and on Friday 9:00am - 12 noon. Simply call to schedule an appointment at: 972.820.9333. We know that sometimes emergencies do occur. If you find yourself needing to cancel or reschedule your appointment notify us as soon as possible so your appointment time will be available for other patients desiring to be seen. You will receive a reminder call of your scheduled appointment 2 days in advance. We appreciate a 24-hour advance notice of cancellations or requests to reschedule your appointment to avoid a $30.00 charge to your account.
PRESCRIPTIONS
It's easiest to request prescriptions and refills during your office visit.
If you need a prescription refill between visits, please have your pharmacy fax a refill request to our office at: 972.492.8600.
Please remember that in certain situations, good medical care requires the benefit of a patient examination before some prescriptions are issued.
Timesaver tip: Be good to yourself and minimize frustration by calling your pharmacy ahead of time to make sure the prescription is ready for you to pick up! Additionally, please pick up your prescriptions as soon as they are available because some pharmacies restock prescriptions that are not retrieved within 3 days.
Your insurance contract is an agreement between you and your insurance carrier. Our practice participates with most major insurance carriers. As a service to you, we will file your claims with your insurance carrier and will do our very best, based on the information you and your insurance company provide to us, to notify you of any financial responsibilities prior to your visit. To provide you with this information, we must have all insurance information prior to your appointment time including any secondary insurance information. We always encourage you to contact your insurance provider if you have questions regarding any financial obligations or coverage. Due to the time necessary to obtain this information, please be aware that if you have not provided up-to-date insurance information prior to your appointment, you may be asked to reschedule your appointment. · Contact your insurance carrier prior to your annual well-woman examination for any policy limitations. · Bring your insurance card and photo I.D. to every visit. · If your insurance carrier requires a referral from your primary care physician, please be sure to bring your referral to your appointment. · Contact your insurance carrier prior to your visit to ensure that your physician is included in their network. Global fees for obstetrical care include routine prenatal care, vaginal delivery and post partum visit(s). Additional fees are charged for diagnostic services such as lab tests, problem visits un-related to pregnancy, and ultrasound examinations, as well as newborn circumcisions. If you ever have any questions, our billing and insurance specialists are experienced and willing to help.
FINANCIAL RESPONSIBILITIES
As required by insurance carriers, you are responsible for the payment of deductibles, co-payments and non-covered services. Payment of such is due at the time of your office visit unless prior arrangements have been made. For your convenience, we accept cash, VISA, MasterCard, American Express and Discover for payment. Please note that our office no longer accepts checks.
You will receive separate bills from all providers of services, including laboratory, x-ray, pathology, anesthesia and the hospital. Please be aware that this office does not verify benefits for laboratory studies nor for any services your doctor recommends which are performed outside the office. It is your responsibility to contact your insurance company if you have questions regarding any financial responsibilities for these services or regarding bills from other providers.
We are partners together in your healthcare. If you have a question or do not fully understand why a certain test or treatment is recommended by your doctor, feel free to ask! We are more than happy to spend the time necessary to ensure your questions are answered.
The practice charges an administrative fee for the completion of medical forms, such as FMLA / medical disability, insurance underwriting applications, and life insurance applications.
