We know your time is very valuable.  In order to shorten your stay in the office and maximize your time with the doctor, we request that you download your patient forms, review and complete them PRIOR to coming in for your appointment.  If you have any questions regarding any of the forms, please contact the office at 972.820.9333.  We will be happy to assist you.

Please note, if you do not complete the forms PRIOR to your visit, you will need to arrive 25 minutes before your scheduled appointment time.
